Clinical services will be charged to you based upon the following Fee Schedule. Session length is determined at the clinician’s discretion. Please note that telephone conferences and court related clinical services may be invoiced at the appropriate regular fee and are not covered by insurance.
Please note that while we may accept payment from many different insurance carriers, we may not be in-network with all of them. As a courtesy, we will bill your in-network insurance company. Please be aware that there are many insurance companies and the clinician reserves the right to determine whether the office will bill your out-of-network insurance company or provide you with a receipt of services to submit on your own accord. Insurance coverage is a contract between the patient and their insurance carrier. We will assist you in maximizing your insurance benefits by verifying your benefits before the initial date of service; however, please be advised that this is not a guarantee of payment. It is the responsibility of the patient to know and understand the benefits of his/her insurance plan.
Please check your coverage carefully by asking the following questions to your insurance provider:
- Do I have mental health insurance benefits?
- What is my deductible and has it been met?
- How many sessions per year does my health insurance cover?
- What is the coverage amount per therapy session?
- Is approval required from my primary care physician?
- Is pre-approval necessary before initial sessions?
If you do not show up for your scheduled therapy appointment, and you have not notified us at least 24 hours in advance, you will be required to pay the full cost of the session.
